Use this form to report accessibility issues or concerns in campus programs, facilities, or grounds to the Office of ADA Compliance.

Compliance Hotline

This is the university’s main compliance reporting mechanism. Use this form to report potential legal, policy, or ethical conduct violations or concerns to the university’s Central Compliance Office.

Users may remain completely anonymous throughout the process.

Disability Discrimination

Use this form to report incidents of disability-based discrimination to the Office of ADA Compliance.

CONSTRUCTION SITE HARASSMENT

Use this form to report sexual harassment at campus construction sites to the Title IX Compliance Office.

REPORT CHILD ABUSE

Step 1: Report known or suspected child abuse, neglect or abandonment (including physical, sexual and/or emotional abuse) that occurred on university property or at a university-sponsored event directly to the Department for Children and Families (DCF).

Step 2: After the report is made to DCF, notify your University supervisor and submit an incident report to UF Compliance and Ethics via the Incident Report form.

REPORT SAFETY OR ENVIRONMENTAL CONCERNS

Use this form to report campus safety or environmental concerns or issues to the Department of Environmental Health and Safety.

UPD SILENT WITNESS

Anonymously report incidents of crime or disruptive behavior occurring on campus to the University of Florida Police Department.

UPD CITIZEN COMPLAINT

This form is for reporting incidents of  mistreatment or inappropriate behavior by an employee of UF Police Department, or dissatisfaction with the quality of service provided by our UPD.

REPORT Sexual Misconduct and Harassment

Report incidents of gender-based discrimination, harassment, or violence. This includes sexual assault, sexual exploitation, sex discrimination, sexual harassment, dating and domestic violence, retaliation, and stalking to the Title IX Compliance Office.
