CARE Concerns allow students, faculty, staff, and families to report a concern for a UF student. If you are aware of a student who is experiencing difficulties, please use this form to let the CARE Office know.

This form is used to report behavioral misconduct of students as outlined in section 4 of Regulation 4.040, the Student Conduct Code, to Student Conduct and Conflict Resolution.

This form is for reporting incidents of academic integrity misconduct that violate section 3 of Regulation 4.040, the Student Honor Code. This form is specific for faculty to report Honor Code incidents in academic coursework, to Student Conduct and Conflict Resolution.

This form is for reporting incidents of disability-based discrimination to the Office of ADA Compliance.

Report incidents of sexual misconduct, discrimination, harassment, or violence. This includes sexual assault, sexual exploitation, sex discrimination, sexual harassment, dating and domestic violence, retaliation, and stalking to the Title IX Compliance Office.

Anonymously report incidents of crime or disruptive behavior occurring on campus to the University of Florida Police Department.

This form is for reporting incidents of  mistreatment or inappropriate behavior by an employee of UF Police Department, or dissatisfaction with the quality of service provided by our UPD.

Use this form to report sexual harassment at campus construction sites to the Title IX Compliance Office.
